The same woman three times: before the shot, a year later and much slimmer, and holding a handful of hair over the bathroom sink.
The weight came off. Then the hair started coming out.
In 30 seconds
  • Shedding is extra hair falling out after fast weight loss. That hair usually grows back once your weight holds steady.
  • Thinning only on top, with the back still thick, is a different kind. It doesn’t settle by itself.
  • A hand mirror shows which one you have. Look at the top, then the back.
  • 5 things women try for it, checked on the same 5 points. One gets all 5 right.
The short answer

Usually, yes. Shedding settles a few months after your weight holds steady. Then the hair fills back in.

Thinning only on top, while the back stays thick, is a different kind. That one doesn't settle by itself.

A hand mirror shows which one you have. For thinning on top, see number one in the ranking below.

When your hair starts coming out on a GLP-1, you want to know if it will grow back.

Hair you lose to shedding usually grows back. Thinning on top doesn’t settle by itself. It helps to know which one you’re seeing.

On the shot now? The difference is easy to see once you know where to look. About to start? You'll know what to watch for.

What's going on

Fast weight loss makes lots of hair fall out at once, all over. That's shedding, and it ends by itself.

Thinning on top has a different cause. It comes down to two hormones.

Your body makes two hormones that matter for the hair on top. Estrogen is one. Every woman makes it, and one of its jobs is guarding the hair roots.

The other is DHT. Your body makes it from testosterone, which women make a little of too. DHT shrinks the hair roots on top, so each new hair comes in thinner.

The two pull against each other. While estrogen is up, DHT loses. When estrogen drops, DHT wins on top.

Whiteboard: a green stick figure labelled estrogen holds a shield in front of a hair root, and a red spiky ball labelled DHT charges at it. Estrogen guards the roots; DHT shrinks the roots. Written below: when estrogen drops, DHT wins on top.
Estrogen guards the roots on top. DHT shrinks them. When estrogen drops, DHT wins.

On a GLP-1, that happens in three steps.

  1. 1
    Estrogen guards the top

It keeps DHT from shrinking the roots on top.

  2. 2
    The shot takes fat fast

Part of your estrogen is made in fat, so less fat means less estrogen.

  3. 3
The top thins. The back doesn't.

DHT reaches the roots on top, and each new hair grows in thinner.

Illustration in three rows: estrogen guards the hair roots on top against DHT; the shot takes fat fast and part of your estrogen drops with it; the top thins while the back stays thick.
Less fat means less estrogen. The top thins, and the back stays thick.
Shedding ends by itself. Thinning on top comes from DHT, and it doesn't end by itself.

How you got here

The shot worked. The scale finally moved, maybe faster than you hoped.

Then, around month 3 or 4, the brush filled up. There were clumps in the shower, and your ponytail felt thinner.

A hairbrush full of long brown hair on a bathroom counter in early morning light.
Around month three, the brush fills up faster.

Everyone had advice. Eat more protein, take biotin, wait it out.

Most of that is true. But nobody said how to tell if what you're seeing is only shedding.

How will I know if it's not the 'usually reversible' type?

How to tellThe mirror test

  • Where it shows. Shedding thins your hair all over. Thinning on top shows at your part and the crown of your head.
  • The back of your head. Hold up a hand mirror. If the back is thick and the top is thinner, that's thinning on top.
  • How it comes out. Shedding comes out in handfuls, then slows down. Thinning on top is slow. Your part just gets a little wider over time.
  • How it feels. Frizzy or dry hair is common on the shot. That's the hair itself. Thinning on top starts down at the root.
Whiteboard with two drawn heads: the shed, hair thin all over with loose hairs falling; the top, thick at the sides but the part wide open with red DHT stars. Written below: on a GLP-1 you can have both.
Shedding thins it all over. The other kind thins only the top. You can have both.

The two kinds can happen at the same time. Sometimes heavy shedding makes thinning on top easier to see. The thinning may have started earlier, before you noticed.

Still shedding 6 to 9 months after your weight holds steady? See your doctor, and ask about iron, thyroid and vitamin D.

Whiteboard timeline: first shot; month 1-3, shedding starts; month 3-6, the worst of it; month 6-9, it usually settles. Written below in red: the top doesn't settle by itself.
Shedding usually settles by month 6 to 9. Thinning on top doesn’t settle by itself.

The 5 things that matter

A hand holds a supplement bottle. Its label shows five gold checks: DHT ingredient in mg, saw palmetto in mg, every dose printed, low biotin and no iodine, two a day.
The front says hair. The back tells you what’s in it.

What to look for

Here’s what to look for on the back of the bottle.

  1. Made for women. It says women on the front, not just hair, skin and nails.
  2. The DHT job. It has pumpkin seed oil and saw palmetto, with the amounts. Saw palmetto is a palm berry. Both plants have been studied for slowing DHT.
  3. Form and dose. Two a day or less. Each ingredient shows its own amount, not one total for a “blend.”
  4. Easy on blood tests. Biotin under 1,000 mcg and no iodine, so your blood tests stay accurate.
  5. 12 weeks, fair price. Hair needs about 12 weeks to show a change. Those 12 weeks should cost about $60 or less.

How it works. Estrogen helps guard the roots on top. On the shot, you lose some of it along with the fat.

Women make a little testosterone too. At the root, a natural helper called an enzyme turns some of it into DHT.

DHT is the hormone that makes each new hair on top grow in thinner.

Both plants slow that enzyme, right at the root. So less DHT gets made, and less of it reaches the roots on top.

Whiteboard drawing: testosterone goes through an enzyme and becomes DHT, which reaches the hair root. Pumpkin seed oil 1,000 mg and saw palmetto 320 mg point at the enzyme, and a green bar half closes the way to DHT. Written below: less DHT reaches the root.
Both plants slow the enzyme that makes DHT, so less of it reaches the root.

Each plant slows it in its own way. No prescription needed.

Pumpkin seed oil is pressed cold from the seeds, like a good olive oil. Saw palmetto is the berry of a small palm that grows wild in Florida.

There is one trial, and it was in men. They took 400 mg of pumpkin seed oil a day for 24 weeks. At the end, their hair count was up about 40%.

There's no trial in women yet. Number one has 1,000 mg a day.

It's the only one here with 1,000 mg cold-pressed pumpkin seed oil and 320 mg saw palmetto. The biotin is kept low, at 100 mcg, and there's no iodine.

It's made for women losing hair on top, on the shot or about to start.

Now check yours

Already taking something? Check the back of the bottle for these five things. Not yet? Use them on the next one you pick up.

  • Does the front say women?
  • Are pumpkin seed oil and saw palmetto both on it?
  • Is every amount printed, with no blend?
  • Is biotin under 1,000 mcg, with no iodine?
  • Do 12 weeks cost about $60 or less?

Count how many it gets right.

If yours gets only one or two, that's not a mistake you made. You picked it off the same shelf everyone does.

What women ask about shedding

Is GLP-1 hair loss permanent?

The shedding usually isn’t. It settles once your weight holds steady. The hair fills back in over the next few months. Thinning on top is the kind that can stay.

How much hair loss is normal?

Losing 50 to 100 hairs a day is normal. Shedding on the shot can be a lot more, all over, for a few months. Thinning on top is slower. Your part slowly gets wider.

When should I see a doctor?

If it’s still coming out 6 to 9 months after your weight holds steady. Or if you see round bald patches. Ask about iron, thyroid and vitamin D.

Can I take number one with my shot?

Number one is two plant extracts, three minerals, vitamin E and a little biotin. Show the label to your pharmacist first, especially if you take hormones, birth control or a blood thinner.

When would I see a difference from number one?

You won’t see much in weeks 1 to 4. Check your part at week 8. Decide at week 12.

Or you close this page. That's allowed.

Nothing happens. The shedding settles, and every few weeks you check your part in the mirror.

You can tell the two kinds apart now.
